To request certified copies of a birth or death record, please see the appropriate information below and complete the request form.
Birth Certificates
Certified copies of birth certificates may be obtained at the City Clerk’s office for any birth occurring in the City of Sterling Heights (e.g. home births). Government issued identification of a person named on the record is required. Please include a copy when ordering by mail along with your check or money order.
Death Certificates
Certified copies of death certificates may be obtained at the City Clerk's Office for any death occurring in the City of Sterling Heights
Fees
- $26 - First Certified Copy (includes filing fees)
- $9.50 for each additional certified copy

We accept checks or money orders for mailed requests as well as credit card or cash for in-person requests.
